1. The indexing head is operated incorrectly, if the indexing fork is not used correctly; the handle is not evenly rotated in one direction; when the indexing handle is accidentally rotated too much, the gap between the indexing head and worm pair is not eliminated.
2. The workpiece is not well calibrated, resulting in excessive radial runout of the workpiece.

The milling cutter is not centered.

1. The depth of the milling layer is adjusted incorrectly.
2. Wrong choice of milling cutter modulus or tool number.
